HIPAA Risk Assessment Is More Than a Checklist or Scan, DYOPATH Guide Says

Oct 22, 2026

A new DYOPATH guide explains why a checklist, questionnaire, or vulnerability scan falls short of a HIPAA security risk assessment, and what a proper one covers.

Oakbrook Terrace, United States, October 22, 2026 /NewsNetwork/ -- DYOPATH has published a guide to HIPAA security risk assessments (https://dyopath.com/hipaa-security-risk-assessment/), aimed at healthcare organizations that may have completed something they call an assessment without meeting the actual requirement.

Many organizations have done a checklist, a vendor questionnaire, or a firewall check and called it finished. The guide says that doesn’t meet the requirement. Office for Civil Rights audits and breach investigations tend to focus on the difference.

The rule behind it is 45 CFR § 164.308(a)(1)(ii)(A), the risk analysis requirement in the HIPAA Security Rule. It applies to any covered entity or business associate that holds electronic protected health information, whatever its size.

According to the guide, the assessment must extend well beyond the network. It looks at administrative safeguards like training and business associate agreements, physical safeguards like facility access and device disposal, and technical safeguards like access controls and encryption. Scans mostly test that last group, which is why one scan can’t stand in for the whole analysis.

The guide puts the inventory first, meaning where patient data actually lives. It points to backups, fax servers, billing platforms, and texting apps as places that get missed. After that, it scores each risk and tests the controls already in place.

No pass or fail grade comes out of it. The guide describes a prioritized risk register and a remediation plan, and says the Office for Civil Rights often asks for the assessment first in an investigation.

The gaps assessors find most often include patient data nobody tracked, access left open after a role change, and lapsed business associate agreements.

HIPAA has no fixed interval for redoing it. Many organizations review annually, and a merger, a new electronic health record system, or a security incident is reason to reassess sooner.

The larger message is that compliance is ongoing. No assessment, and no vendor, can make healthcare organizations (https://dyopath.com/healthcare-medical/) “HIPAA compliant” as a one-time achievement. Findings need owners, deadlines set by severity, and tracking until they’re closed.

DYOPATH says it has performed assessments against HIPAA, NIST, and related frameworks through its governance, risk, and compliance practice. Organizations that want to discuss a cybersecurity assessment (https://dyopath.com/cyber-security-assessments/) can contact the team.

Local figure skating competition Halloween Cup goes international

For its second year, recreational figure skating competition SM Skating Halloween Cup, hosted several international participants for the first time, making the local contest into an international meet. “So this is the second year we hosted the Halloween Cup. This year, the difference is it's an international competition. The first year that we hosted it we had over 100 participants, but this year almost 180 participants,” explained Nikki Cheng, president of the Philippine Skating Union. More than 180 figure skaters from the Philippines, Mongolia, Thailand, and the United States, participated in the event, which was held from October 16 to 18 at SM Mall of Asia in Pasay City. Wearing various costumes, from Disney princesses to K-pop idols, the young skaters showcased their skating skills and fun routines. For the first, the competition also joins the Ice Skating Institute Asia (ISIAsia) Championship Series. According to the competition, the event was not just a “thrilling Halloween spectacle” but it “champions SM Active's mission to make sports accessible, nurture young talent, and inspire more Filipinos to embrace an active lifestyle”. “We feel that it is very important for its contribution to grassroots development. We see that the sport is continually growing. In this event we get to discover who are the ones that we would bring into the national developmental [program]. This is a recruitment stage for us,” Nikki shared. Check out the photos from this year's event in the gallery below: Love this content?
